PARTICIPANT CONSENT FORM

 

Thank you for your participation in this Prescreening and General Survey study. This research is being conducted by faculty, graduate students, and honors students in the Department of Psychology here at the University of California at Berkeley, and is supervised by Professor Oliver John, Chair of the Research Participation Program (RPP) Committee of the Psychology Department.


We hope that you will find this study interesting and educational. The main benefit of the research is that it will contribute to an understanding of how people see and describe the way they think, feel, and behave in a variety of situations. There are no known risks to participating in this study. Research reports based on these data will not publish individual data, but will describe data in aggregate forms (e.g. means, correlations).


You understand that you will be asked to fill out a number of questionnaires online that will take approximately one hour to complete. Questions concern demographic background information as well as your views and perceptions of yourself and others. These questions are generally not considered sensitive and do not involve content that would be significantly embarrassing if revealed.


You also understand that your participation is voluntary and that you are free to withdraw your consent and discontinue participation in this study at any time by logging out or closing your browser.  However, by not submitting your survey, you forfeit the one-hour course credit you would receive upon completion. If you choose to participate in this study, you may refuse to answer any question(s) that might make you feel uncomfortable and still receive full credit by clicking through to the end of the survey and selecting "Submit Survey". PLEASE NOTE: You must complete the survey in one sitting. There is no option to save and continue; if you close your browser, you will have to start over from the beginning. As well, if you do not complete the survey by the deadline, you will not receive credit.


You understand that your answers are confidential, and that the information you provide will be used only for research purposes. You will be asked to provide your Student Identification Number (SID) for CalNet authentication purposes. However, your survey responses will not be linked to your SID, and researchers will not have access to your SID. Instead, your survey will be assigned a randomly generated code.  In a separate file, this code will be linked to your name and email address in order for researchers to contact and invite you to participate in studies offered during this semester as part of the Psychology Department Research Participation Program. It is possible that some undergraduate students who you may or may not know will have access to your identifiable data in the event that you qualify for participation in research associated with their honors theses. You may refuse to give permission for this use of your data and still participate in this study for full credit by selecting the 'Do use my data' option at the end of the survey. Once you have made this request, you will not be contacted by researchers.

 

 Because only about 10% of the RPP researcher recruit participants via email, it is quite possible that you will not be contacted during the semester. Thus, you should not wait to be contacted for research projects requiring screening but rather sign up for any project in which you may wish to participate.
